Patologin lausunto

Lisätäänpä tänne vielä tarkemminkin se patologin lausunto (sain tämän maanantaina), kun epäilen, että sähköposti (missä tämä lukee) hukkuu jonnekin joten tämä saattaa olla paremmin tallessa täällä :)



COMMENT - Note
Histopathology and cytology results can be discussed
Monday to Friday, 11:00 h to 16:00 h.
MACROSCOPY:
Submitted are two skin covered tissue samples; 
one measuring 19 x 11 x 3 mm,  
the other one 29 x 14 x 11 mm.

HISTOLOGY:
Smaller sample; in several section levels small rather nodular appearing 
change within the mid dermis appears which consists ofproliferating 
collagen fiber bundles with embedded hyperplastic sebaceous glands, 
dilated sweat glands and follicular hyperkeratotic hair follicles. 
Surrounding tissue is partly compressed, but otherwise unremarkable.
No significant inflammatory response.

MARGINS:
Complete removal of the small nodule.

Larger sample:

Dermis; expansive growing well-delineated but non-encapsulated mass 
starting in the superficial dermis and reaching into the deep dermis 
consistent with a multilobular proliferation of well-differentiated 
glandular epithelium arranged in groups of cytoplasm with eosinophilic 
cells with central monomorphic nuclei surrounded by a thin rim of reserve 
cells with very few mitotic figures (less than 1 per ten HPFs). 
In between partly large blood-filled spaces with endothelial lining. 
In the surrounding tissue some unchanged appearing perianal glandular tissue 
observed.

MARGINS:
Complete removal with security margin achieved.

DIAGNOSES:
Smaller sample; fibroadnexal dysplasia.
Larger sample; perianal gland adenoma.

BEHAVIOR:
Both benign.

PROGNOSIS:
Both favorable.

COMMENT:
Fibroadnexal dysplasia is a hamartoma, clinically observed are these 
lesions in most cases in the last third of the life span. They are there 
already from birth, but it takes some time to develop to visible size. 
They can appear multifocally.

Perianal gland adenoma from somewhere near the perianal region in the 
female with complete removal as in this case carries an excellent prognosis. 
Relapse is not to be expected.
